The following form may be utilized to keep track of loss of income due to unauthorized copying of e-filed transcripts in the State of Kansas.  This information will be compiled and maintained by the KCRA Board.  No information of an individual reporter will be disseminated beyond the board without notification and express permission of the contributing reporter.  Send all completed forms to jenolsenrpr@gmail.com.
